NAME OF APPLICANT / आवेदक का नाम Please enter your full name exactly as mentioned on your PAN Card. / कृपया अपना पूरा नाम पैन कार्ड के अनुसार दर्ज करें।
EMAIL ID / ईमेल आईडी Enter a valid and active email address for communication purposes. /कृपया एक वैध और सक्रिय ईमेल आईडी दर्ज करें।
MOBILE NUMBER / मोबाइल नंबर/Enter your 10-digit mobile number. Make sure the number is active as it will be used for OTP verification./कृपया अपना 10 अंकों का मोबाइल नंबर दर्ज करें। सुनिश्चित करें कि यह सक्रिय हो, क्योंकि इसी पर ओटीपी सत्यापन किया जाएगा।

NAME OF THE FOOD CATEGORY / खाद्य श्रेणी का नाम/Select the appropriate food category applicable to your business from the available options./दिए गए विकल्पों में से अपने व्यवसाय के अनुसार उपयुक्त खाद्य श्रेणी का चयन करें।
KIND OF BUSINESS / व्यवसाय का प्रकार / Choose the type of business you are operating from the provided list. / प्रदान की गई सूची में से अपने व्यवसाय का प्रकार चुनें।
COMPLETE BUSINESS ADDRESS / पूरा पता / Provide the complete address of your business location. / अपने व्यवसाय का पूरा पता दर्ज करें।
PINCODE / पिन कोड /Enter the correct pincode of your business address. /अपने व्यवसायिक पते का सही पिन कोड दर्ज करें।
STATE / राज्य / Select the state where your business is located from the dropdown menu. / ड्रॉपडाउन मेनू से अपने व्यवसाय का राज्य चुनें।
DISTRICT / जिला / Select the district where your business is situated from the dropdown menu. / ड्रॉपडाउन मेनू से अपने व्यवसाय का जिला चुनें।
UPLOAD PAN CARD / पैन कार्ड अपलोड करें / Click on the “Choose File” button to upload a clear scanned copy of your PAN Card. / “Choose File” बटन पर क्लिक करके अपने पैन कार्ड की स्पष्ट स्कैन कॉपी अपलोड करें।
TERMS OF SERVICE / सेवा की शर्तें / Please read the Terms of Service carefully and tick the checkbox to indicate your agreement. / सेवा की शर्तों को ध्यानपूर्वक पढ़ें और सहमति देने के लिए बॉक्स पर टिक करें।
OTP CONSENT / ओटीपी सहमति / Tick the checkbox to confirm that you agree to share the OTP sent to your registered mobile number for verification purposes. / सत्यापन के लिए आपके मोबाइल नंबर पर भेजे गए ओटीपी को साझा करने की सहमति हेतु बॉक्स पर टिक करें।
VERIFICATION CODE / सत्यापन कोड/ Enter the 5-digit verification code shown below in the given field. / दिए गए फ़ील्ड में नीचे प्रदर्शित 5 अंकों का सत्यापन कोड दर्ज करें।

🟢 Basic FSSAI Registration
For small food businesses with an annual turnover up to ₹12 lakh.
Suitable for home kitchens, small retailers, petty food manufacturers, and startups.
🟡 State FSSAI License
For medium-sized food businesses with an annual turnover between ₹12 lakh and ₹20 crore.
Required for restaurants, medium manufacturers, storage units, transporters, and distributors operating within one state.
🔴 Central FSSAI License
For large food businesses with an annual turnover above ₹20 crore.
Mandatory for importers, exporters, large manufacturers, and businesses operating in multiple states.
